Angelica Ministries, LLC is developing services for individuals ages 5 and older who may:
Have a diagnosed mental health disorder or experience symptoms of a behavioral health crisis.
Experience significant emotional, psychological, or behavioral distress that affects daily functioning.
Be at risk of psychiatric hospitalization, emergency department utilization, homelessness, family disruption, school failure, or loss of community placement.
Need crisis assessment, intervention, stabilization, and coordination of ongoing behavioral health services.
Be appropriate for community-based crisis support and monitoring when it is safe and clinically appropriate.
Our approach is person-centered, trauma-informed, culturally responsive, and focused on helping individuals and families achieve safety and stability while remaining connected to their communities whenever appropriate.
When services are established, Angelica Ministries, LLC may assist individuals and familes in accessing a higher level of care when:
A medical condition requires immediate emergency medical treatment.
The individual presents a level of risk that cannot be safely managed in a community-based setting.
Inpatient psychiatric hospitalization is clinically necessary.
The individual's needs exceed the scope of services offered by the agency.
Safety and well-being are central to Angelica Ministries, LLC's appraoch to care. When a higher level of care is needed, appropriate referrals and ccontinuity-of-care support may be provided within the scope of the agency's establsihed services and applicable requirements.
When services are established , an individual, family member, school, healthcare provider, or community partner may contact
Angelica Ministries regarding a behavioral health crisis.
A qualified member of the service team will the individual's immediate needs, safety concerns, and level of support required, consistent
with applicable requirements.
When appropriate and within the scope of established serves, the mobile team may provide crisis intervention and stabilization in the individual's community setting.
The service team may works with the individual and appropriate supports to develop a plan focused on safety, stabilization, and ongoing needs.
When additional services are needed, appropriate referrals and continuity-of-care support may be provided within the scope of established services and applicable requirements.
Angelica Ministries, LLC is devloping a person-centered, trauma-informed, culturally responsive, and recovery-oriented approach to mobile mental health crisis support.

Safety: Promoting the immediate safety and well-being of the individual, family, and community.
Stabilization: Supporting efforts to reduce the intensity of the crisis and promote emotional and behavioral stability.
Individualized Support: Developing interventions based on each person's needs, strengths, preferences, and circumstances.
Family & Natural Supports: Involving family members and other appropriate supports when authorized and clinically appropriate.
Community-Based Care: Supporting services in natural community settings whenever safely possible and clinically appropriate.
Connection to Ongoing Care: Helping individuals connect with appropriate behavioral health, healthcare, and community resources following a crisis, when applicable.
Crisis Assessment
When services are established, Angelica Ministries, LLC will use an individualized assessment process to consider the person's current needs, symptoms, safety concerns, strengths, and available supports.
Crisis Intervention
When appropriate and within the scope of established services, qualified team members may provide immediate, person-centered interventions designed to reduce emotional and behavioral distress and address the circumstances contributing to the crisis.
Stabilization
The service model is designed to provide short-term support to help individuals regain stability and remain safely in their homes and communities whenever clinically appropriate.
Safety Planning
When appropriate, individuals and their families may participate in developing individualized safety plans that identify warning signs, coping strategies, supports, and steps to take if t a crisis escalates.
Care Coordination
When services are established, Angelica Ministries LLC, may work with families, schools, healthcare providers, community partners, and other appropriate supports to coordinate services and promote continuity of care.
Referrals and Follow-Up
When additional services are needed, appropriate referrals and coordination with ongoing behavioral health and community resources may be provided within the scope of established services and applicable requirements.
Angelica Ministries, LLC recognizes that every individual and family has unique needs, strengths, experiences, and circumstances. Our developing crisis service model is designed to support each person's individual needs while promoting safety, dignity, choice, and independence.
When services are established, care and support may be developed with the individual and, when appropriate and authorized, family members, caregivers, and other members of the individual's support network.
The service model is designed to identify strengths and natural supports while connecting individuals with appropriate behavioral health and community resources that may support continued stability beyond an immediate crisis.
